My research focuses on health system elasticity: the extent to which health systems modifications can reduce cervical cancer, HIV, and STD health inequalities. Systems of interest have included 1)STD clinic practice expansion to offer cervical cancer screening 2)Pharmacy practice expansion to include sexual health services such as HPV vaccination HIV testing, consultation and linkage to care; 3)Community Health Centers and their ability to deliver standard of care for HIV and STI screening and management My cervical cancer research has focused on STD clinic adoption of cervical cancer screening and navigation to follow-up colposcopy. Current studies focus on both clinician-examined patients as well as STD "FastTrack" patients (those who are triaged based on risk into providing self gathered samples (NAAT testing) for chlamydia, gonorrhea with a blood draw for HIV and syphilis. We are also just completing a national review of state comprehensive cancer plans to identify 1) prioritization of cervical cancer, 2) the conceptualization of cervical cancer and HPV, and 3) the policy tools planned for use. Studies in development include examination of FastTrack patient acceptance and completion of cervical cancer screening; a comparison of cervical cancer screening outcomes between FastTrack and clinician-examined STD patients; and a study to identify the distribution and nature of cervical cancer screening and of FastTrack services among a nationally representative sample of sexually transmitted disease clinics in the U.S. These studies together will inform the development of larger studies of the impact of STD clinic-based cervical cancer screening and navigation to follow-up (colposcopy) on cervical cancer screening and follow up disparities for uninsured and medically underserved women in the U.S.
