1. Setting up of Anticoagulation clinics: Deep vein thrombosis and pulmonary embolism remain as important causes of morbidity and mortality in patients with or without cancer. Several thousand patients in the Indianapolis and surrounding areas receive anticoagulation with Warfarin and Heparin like drugs to enable them to treat and avoid potentially life-threatening blood clots. Careful control of these patients while they are on these medications to promote efficacy and avoid complications requires constant blood checks with reporting and adjusting of medications. Such repetitive tests are best monitored and responses standardized in Anticoagulation Clinics. I plan to help set up such a facility at the IU Simon Cancer Center and affiliated Clarian facilities. 2. Pharmacogenomic study of Warfarin therapy: Warfarin is a commonly used medication to prevent blood clots. Complications due to too much or too little of an effect of this drug can be hazardous. Recently, it has been known that patients react to Warfarin depending on how they metabolize the drug. This individualized drug response is the basis of Pharmacogenomics. In collaboration with Dr. David Flockhart and colleagues, I plan to start utilizing these tools to predict and promote appropriately dosed drug therapy.
2. Thrombotic Microangiopathies such as Thrombotic Thrombocytopenic Purpure (TTP) and atypical Hemolytic Uremic Syndrome (aHUS)remain enigmatic puzzles of the vascular system. Both timely recognition and treatment of these illnesses is of paramount importance. We have initiated protocols for treatment of aHUS with newer agents targeting the complement pathway, especially so after renal transplantation. Use of these newer agents remain as challenges as regards the duration of treatment and organ survival.
