Pancreatic cancer screening and early detection in patients with pancreatic cystic neoplasms and hereditary forms of pancreatic cancer
Since its creation in 2003, the IU Pancreatic Cyst and Cancer Early Detection Center has identified and followed well over 1000 patients with precancerous pancreatic cystic neoplasms. In 2016, patients with at-risk germline mutations (BRCA1&2, Peutz-Jeghers) and familial pancreatic cancer were added to the surveillance program, creating one of the largest pancreatic cancer early detection programs in the world. We are currently working with gastroenterologists, geneticists and other specialties to create a multidisciplinary clinic for high-risk individuals.
2. Improvement of outcomes after pancreatic surgery
Pancreatectomy is still associated with a 1-2% mortality and a 40% morbidity. I have dedicated much of my research to quality improvement, trying to identify ways to predict and decrease complications associated with pancreatic resection.
3. Use of new technologies to identify patients at higher risk for developing pancreatic cancer
I have past experience in artificial intelligence and use of natural language processing to identify patients at high-risk of pancreatic cancer and improve pancreatic cancer screening in this sub-population.
4. Study of the natural history of patients with necrotizing acute pancreatitis
Necrotizing acute pancreatitis represents 20% of all acute pancreatitis cases. Historically, surgical debridement was the mainstay of treatment for those patients despite a high perioperative mortality risk. Recently the PANTER trial has challenged our management armamentarium for those patients. I have started my necrotizing pancreatitis research by creating our own prospective institutional database of patients with necrotizing acute pancreatitis which allowed us to study its true natural history.
